Request Copy of Case File
Medicare case files are protected by the Privacy Act of 1974, 5 U.S.C. §552a and 42 CFR § 423.136 - Privacy, confidentiality, and accuracy of enrollee records.
Every Medicare enrollee or their authorized representative is entitled to a copy of his or her case. The QIC will provide one copy of your case file free of charge, but may charge for any additional copies. If you would like a copy of your file, please contact C2C by making a written request.
Freedom of Information Act The Freedom of Information Act (FOIA), Title 5 of the United States Code, Section 552, provides any person the right to request access to non-privileged records and documents maintained by Federal agencies, and those acting on behalf of Federal agencies, e.g., Medicare contractors, and receive a response to the request. Third parties, without the written consent of the enrollee, must make a FOIA request to the QIC in writing.
